Allegheny County police are investigating a head-on collision that left one dead and four others with minor injuries late Wednesday afternoon in Fawn.

Barbara Daniels, 67, of West Deer, was pronounced dead at the scene of the crash, which occurred about 5:25 p.m. Wednesday, according to the Allegheny County medical examiner's office.

Fawn police Chief Tim Mayberry said Ms. Daniels was traveling west on Route 908 in a 2012 Chrysler sedan when she crossed into the eastbound lane. She struck a 2010 Chrysler van driven by Christine Windows, who was driving with her three children.

The collision injured Ms. Windows' arm. She was taken to UPMC St. Margaret's by private vehicle.

Her children were taken to Children's Hospital of Pittsburgh of UPMC by private vehicle. Chief Mayberry said their injuries were minor.

Chief Mayberry said county police are investigating why Ms. Daniels crossed into the wrong lane, but he believes excessive speed may have played a role.
