State officials have downgraded the license of the Presbyterian SeniorCare nursing home in Oakmont as a result of a patient's death there in September.
The Willows, as the 202-bed, nonprofit facility is known, will be on a six-month provisional license through March 28 due to the death of Loretta Benzel, 89, on Sept. 24.
Mrs. Benzel, formerly of Plum, committed suicide by crawling out her third-floor window and falling 32 feet, according to the Allegheny County medical examiner's office.
A Pennsylvania Department of Health report made public this week said Mrs. Benzel previously made suicidal threats Sept. 19, but that the nursing home failed to act to protect her safety in the ensuing five days. A Presbyterian SeniorCare spokeswoman said the facility is disputing the finding that it took no action, and has filed an appeal with the health department.
No fine was assessed against the nursing home.
